I understand that I will have to sign a release form before every training service received. I understand that I will have to provide feedback on the training service received. I agree to arrive on time for any training appointments and to provide at least 24 hours notice in the event that I need to cancel or reschedule my appointment. I understand that the failure to provide adequate notice or no-show for my training appointment will result in elimination from the training program and my ineligibility to participate in any future trainings at Eve or Haven. I understand that all services will be performed by licensed technicians in a training environment which may result in multiple viewers and/or technicians in the room during the service. If I am receiving a training waxing service, I attest that I am not taking any medications topically or orally that contain retinol such as Retin-A, Differin or Accutane. I understand that in order to receive a training waxing service, I must allow 4 weeks of hair growth before the training waxing appointment. I will be contacted 4 weeks prior to a training appointment whenever possible to allow time to prepare. Occasionally I will also be contacted on shorter notice and will be booked if I meet the criteria at that time. I will not be penalized if I do not meet the criteria or am unavailable for a training service. I understand that waxing during the menstrual cycle is possible but is often more painful and reactive than at times not during the menstrual cycle. No facial waxing can be performed on the same day as a facial service. No body waxing can be performed on the same day as a body treatment. I agree to arrive for my training appointment clean. I understand that no training massage or body treatment appointment may be performed during pregnancy. Edema is contraindicated for nail services and cannot be performed on clients with that condition.
